Illawarra In-Home Support

What is it?

Illawarra In-Home Support provides Community Aged Care Packages to frail aged people living in the Kiama, Shellharbour and Wollongong Local Government Areas. A Community Aged Care Package is equivalent to low-level hostel care provided in a person's own home. A Package coordinates a range of services tailored to the client's individual needs and can include:

  • housework
  • shopping
  • meal preparation
  • laundry
  • transport
  • socialisation
  • in-home respite
  • medication monitoring
  • personal care
  • low-level garden maintenance

The average package is 6 hours per week of care.

Who is it for?

Community Aged Care Packages are for frail, aged persons who have been assessed by an Aged Care Assessment Team (ACAT) as needing low level hostel care. We provide the equivalent of hostel care in their home. We cannot accept self-referrals.

Client charges are according to Commonwealth Department of Health and Aged Care guidelines. Maximum Fee is 17.5% of single pension. Contributions for service are negotiated with the client when services are organised and no one is refused service due to an inability to pay.

Personal Care Assistants, Assistants in Nursing and Enrolled Nurses are employed across the three Local Government Areas to provide the care.

The Illawarra Aged Care Assessment Team based at Port Kembla holds the centralised Illawarra Community Aged Care Packages waiting list. To book an assessment please contact them on (02) 4223 8365.
